CCDS update: Reminder about the recertification application

A reminder those going through the CCDS recertification process that the application is two pages in length and you must submit both pages when you recertify.

The cover page requires your contact information, full address, and your email address. We do not assume that any information currently on file is your correct, up-to-date, information.

The second page is for the listing of continuing education units (CEU) you are claiming. Read it carefully and note you need to list the program title and sponsor of each program you claim, along with the location where the training took place, the dates, and number of CEUs. You may list “online” as the location for training completed in that manner.

We cannot process incomplete applications and will send them back to you to be reworked as necessary.
