CDI staff help LTAC clinical team tell the patient care story

How can we provide quality care for our patients if we do not have a clear picture of their clinical story? That has been a core question of mine for as long as I can remember, even before I heard the term “clinical documentation improvement.”

We all want to see positive outcomes. But how do the healthcare providers—from the bedside nurses to the dietitians to supporting physicians—know what that care is if it was not clearly documented?
