Addressing the 'bad' and 'ugly' in physician documentation

CDI professionals see a lot of physician documentation—not all of it good or pretty. A few annoying and troublesome problems fall into the “bad” or “ugly” categories. Describing a few of these worrisome practice patterns and identifying ways to improve them may help your CDI program. Some problems you’ve likely tried to address in the past. Others may be new areas of concern that you have yet to discover.
