Membership Update: New form for requesting CEUs

ACDIS has new applications for local chapters and vendors to use to request CCDS continuing education units (CEUs). These are editable PDFs. No more writing applications out by hand!

All you have to do is download the form and save it to your desktop. For the local chapter form, click here; for the vendor form, click here. When you want to submit a program for CEUs, follow through these steps:

1. Save the file to your desktop.
2. Fill out the form completely.
3. Save the document again with a new name (we suggest “Chapter name and event date,” such as “MichiganACDIS041517” or NovantCCDSCE041517)
4. Email it to ACDIS for processing (prichards@acdis.org) 

Please remember that ACDIS needs the form 30 days prior to your proposed event for processing.
