ACDIS update: Updated CEU application for non-chapter events available now!
ACDIS has updated its CEU application for non-chapter events, which you can access on our website by going to the “Certifications” tab, choosing CCDS or CCDS-O, and then selecting “CCDS CEUs” or “CCDS-O CEUs” where all the relevant documents are found. You can also find the application here.
Please note that all completed applications should be emailed to ACDIS Editor and Product Coordinator Karla Kozak (karla.kozak@hcpro.com) at least 30 days prior to the start of the event. (For local chapter events, chapter leaders should still get CEU approval by submitting the ACDIS Local Chapter Leadership Request survey 30 days in advance of their scheduled meeting.)
You’ll notice the largest change in section 7 of the application. We now require a timed agenda for the learning activity. Timed agendas should be a brief outline or bulleted list of the major topics that will be covered. Applicants are encouraged to indicate approximately how much of the total time will be spent discussing each topic.
Timed agendas should be submitted with the application as a separate document or in the body of the email sent to Karla Kozak (karla.kozak@hcpro.com). Gathering this information upfront ensures that we can provide you with the most accurate CEU certificate, avoid any confusion, and, ultimately, allow us to better serve the ACDIS community.
For more information on the application process as well as any changes to the form, see the “How to Apply For CEUs” document, available here.
