News: AHRQ releases 2025 PSI update, containing 40 coding changes

The Agency for Healthcare Research and Quality (AHRQ) has released the 2025 Patient Safety Indicators (PSI) updates and revisions.
According to the document, there are approximately 40 discrete changes. Among the changes are:
- PSI-03, Pressure ulcer rate: Five diagnosis codes added related to cardiogenic shock, cardiac arrest, and anoxic brain damage
- PSI-06, Latrogenic pneumothorax rate: 50 codes added related to thoracic surgery
- PSI-08, In-hospital fall associated fracture rate: 58 codes removed related to osteoporotic pathologic fractures
- PSI-09, Post-operation hemorrhage and hematoma rate: Three codes added related to coagulation disorders
- PSI-10, Post-operation acute kidney injury dialysis rate: Two codes added related to partial or total nephrectomy
- PSI-11, Post-operation respiratory failure rate: 41 codes added
“With this software update,” the AHRQ noted, “the PSI software now accounts for ICD-9-CM, ICD-10 CM/PCS, and DRG/MS-DRG codes valid from October 1, 1994 through September 30, 2025.”
