ACDIS update: Send us your templates

CDI professionals review physician documentation but how are departments documenting their own efforts? ACDIS is looking for templates of how CDI professionals document their medical record review efforts for inclusion in the “Policies & Procedures” section of ACDIS’ resources. The goal is to provide CDI departments across the country with templates and examples for how other organizations are documenting their CDI efforts, and give folks a starting place or ideas to revise their current way of tracking this.

Tracking templates might have spaces for CDI specialist to document their efforts and include information such as:

• Principal diagnosis/active issues
• Procedures
• Opportunities
• Queries
